HOW AND WHEN TO ENTER: Each child who registers for this contest and meets all of the eligibility criteria will automatically receive one contest entry, no donation (or any kind of purchase) necessary. Then, every time someone makes a donation of $5.00 or more (in equivalent U.S. currency) to the Lunch for Life campaign featured on www.lunchforlife.org in honor of that entrant, he or she will automatically receive an additional entry for each $5.00 donated in his or her name. Permissible form of entries and final deadline for receipt of entries is as follows: 1) donation by CREDIT CARD through the www.lunchforlife.org processor or electronically submitted through PayPal must be successfully processed by CNCF by 11:59:59 P.M. Pacific Time, December 24, 2007; 2) donation by PERSONAL CHECK made payable to CNCF and delivered to CNCF at the specified address or electronically submitted through PayPal must be received by CNCF by 5:00 P.M. Pacific Time, December 21, 2007, with specified funds actually transferred to CNCF by 11:59:59 P.M. Pacific Time, December 24, 2007; 3) donation by CASHIER’S CHECK or MONEY ORDER made payable to CNCF delivered to CNCF at the specified address must be received by 5:00 P.M. Pacific Time, December 23, 2007; or 4) donation by CREDIT CARD over the phone, through the phone number supplied on www.lunchforlife.org must be successfully processed by CNCF by 5:00 P.M. Pacific Time, December 24, 2006. Entries delivered in any other manner, or after the applicable deadline, or which cannot be successfully processed by CNCF will not generate entries and will not be considered for purposes of this contest. In order for a donation to result in one or more contest entries for an Entrant, the donation must be equal to or greater than $5.00 U.S. currency, and must also specify the name of the Entrant on whose behalf the donation is being made. CNCF and its affiliated entities and sponsors, and their directors, officers, employees, and agents, assume no responsibility for lost, stolen, late, illegible, incomplete, incorrect, unsuccessful, misdirected, damaged, mutilated, altered, tampered with, or postage-due mail or email, and are also not responsible for technical, hardware or software malfunctions, misdirected entries, lost or unavailable network connections or failed, incorrect, inaccurate, incomplete, garbled or delayed electronic or other communications.
ELIGIBILITY AND GUIDELINES: Entrants must be persons under the age of forty (40) at the time of entry, who have been diagnosed with Neuroblastoma by a professionally licensed medical doctor. Entrant applications must be submitted to CNCF by 5:00 PM on December 23, 2006 in order to qualify entrant for this contest. Void where prohibited. Grand Prize will be awarded to Entrant and up to three (3) additional members of Entrant’s Family or friends. In the case of selection as Winner of an Entrant who is deceased or declared unable to travel by a treating physician, Entrant’s responsible parent or legal guardian may choose to accept the travel package for other members of Entrant’s family, or to designate another qualified Entrant as the new Winner. No other transfer of prizes is permitted. No member of Winner’s party may participate in the Grand Prize trip who: 1) has been diagnosed by a physician as unable to travel; and/or 2) does not possess a valid U.S. passport if a non-citizen of the U.S. At least one member of Winner’s party must be an immediate family member of at least 21 years of age, possessing a valid driver's license.
HOW AND WHEN WINNERS WILL BE SELECTED: On or about December 25, 2007, one Grand Prize Winner will be chosen by random selection by computer from a list of all qualifying entries compiled from September1, 2007 through the December 24, 2007 or other applicable deadline. All qualified Entrants will have a chance to win. Odds of winning depend on the number of eligible entries in this Contest. All of the finalists must be willing and able to work with Children’s Neuroblastoma Cancer Foundation on a written and video tape profile of their family for posting online on the www.cncf-childcancer.org and/or www.lunchforlife.org or any other affiliated web sites by January 31, 2007. In the event a winner is deemed ineligible, fails to comply with contest procedures, or is not available to travel during any specified dates, prize will be awarded to runner-up, time permitting. CNCF and its affiliated entities and sponsors, and their directors, officers, employees, and agents, are not responsible for technical failures of any kind, including but not limited to electronic malfunction or damage of any network, hardware or software. If for any reason the contest is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or other causes beyond sponsors' control that corrupt or affect the administration, security, fairness, integrity or proper conduct of the contest, CNCF reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the contest. No responsibility is assumed for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries; or any problems or technical malfunctions of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any e-mail or electronic entry to be received on account of technical problems or traffic congestion on the Internet or at any Web site, or any combination thereof, including any injury or damage to entrant's or any other person's computer related to or resulting from participation in or downloading any materials from this contest.
GRAND PRIZE: Winner will receive a vacation which includes: a) round trip airfare for four people to and from Orlando, Florida and an airport near Winner's residence, as determined by CNCF personnel; b) hotel accommodations for three nights, including two rooms at or around Disneyworld, defined as rooms and taxes only (Winner’s party will be responsible for incidentals); c) round trip ground transportation for four between the Orlando airport and their lodgings at or around Disneyworld; d) four 2-day passes to Disneyworld, including tickets for four to two Disneyworld shows of Winner’s choice. Approximate retail value: $5,000 to $7,500. All expenses not mentioned above are the sole responsibility of Winner’s party. Winner or a member of Winner's Family who will be traveling with Winner must possess a credit or debit card, which may be required to cover any deposits or charges not otherwise included as part of the prize package. Exchange/transfer of prizes: Winner is not entitled to transfer prizes or to obtain substitutes, but Sponsors, in their sole discretion, may substitute prizes of equal or greater value. The sole exception is in the case of a Winner who is deceased or diagnosed as unable to travel by a treating physician, in which case the Grand Prize may be transferred to another qualified Entrant.
HOW FINALISTS AND WINNER WILL BE NOTIFIED; PROOF OF AGE AND IDENTITY; AFFIDAVITS AND RELEASES: Winner will be notified by phone and by an email sent to the email address specified by Winner’s registration form after the contest result is tabulated. If the Winner cannot be contacted at this email address or phone number, or is contacted and does not respond as directed within 14 days of attempted notification, or refuses the prize or is ineligible to accept the prize, CNCF reserves the right to select an alternate Winner or not to award the Grand Prize. Winner’s responsible parent or legal guardian may be required to complete and sign an Affidavit of Eligibility/Compliance with Rules and, except where prohibited, Liability/Publicity Release and Indemnification, in the form provided by CNCF, within 14 days of attempted notification in order to claim the Grand Prize. Such Affidavit may include proof of age, identity, and Neuroblastoma diagnosis by professionally licensed medical doctor. In the likely event that Winner is a minor, all responsibilities of the Winner must be assumed by Winner's responsible parent or legal guardian, or prize will be forfeited and may be awarded to an alternate Winner. CONDITIONS ON ENTRY: By entering this contest, entrants agree to be bound by the terms of these Official Rules and Conditions, and by decisions of CNCF, which are final as to all contest-related matters. In addition, by participating in this contest and accepting any prize that they may win, Entrants agree to release Children’s Neuroblastoma Cancer Foundation and its affiliated and successor entities, its advertising and promotion agencies and prize suppliers, its web site hosting, design, and support providers, and each of their respective officers, directors, agents, representatives and employees (collectively, the "Released Parties") from any and all actions, claims, injury, loss or damage arising in any manner, directly or indirectly, from participation in this contest and/or acceptance, use or misuse of any Prize or portion of any Prize. Entrants also authorize the Released Parties to use their names, voices, likenesses, biographical data, and cities and states of residence in programming or promotional material or on a Winner's list (if applicable) without further compensation unless prohibited by law. CNCF, in its sole discretion, further reserves the right to disqualify any person who tampers with the entry process or the operation of the web site, or who otherwise violates these rules. By entering, entrants accept and agree to abide by the terms of these Official Rules and Conditions. This sweepstakes shall be governed and construed by the laws of Illinois, and the venue for all controversies will be in state or federal court in Bloomingdale, Illinois.
